Step 1: Complete Registration Form

Complete a short online questionnaire requesting information regarding your injury and medical history.

US & Canadian patients will first register here and once registered then select Book Appointment from the options. Our team will reach out via SMS text within the next business day with the required form. 

International patients will begin by completing this form. Once complete, our Consultation Coordinators will reach out to you via email with the next steps. 

Step 2: Collect & Prepare your Medical Imaging & Reports 

Please collect all operative photos, operative reports, radiology reports, MRIs, and x-rays for the affected joint. The medical imaging should have been completed within one year and please send any typed radiology reports that you may have that accompany those images. All images must be postoperative, meaning after any previous surgeries.

If you do not yet have these images, please have the imaging facility use the following guidelines for the Knee, Shoulder, and Foot/Ankle x-rays and MRIs.

Step 3: Send Us Your Medical Imaging & Reports

You can send The Clinic your imaging online or via mail. You may transfer your medical imaging to The Stone Clinic online using our imaging uploader tool Purview Viva. Simply drag and drop the files of your x-ray or MRI from your CD/DVD or thumb drive into the uploader and they will be securely transmitted to the staff at The Clinic.

To have your medical information returned after your consultation, please include a self-addressed FedEx shipping label including your FedEx account number or credit card information for shipment along with your package. 

In order for Dr. Stone to properly view the diagnostic detail of your medical imaging and provide the best possible assessment of your injury, we require that all MRI and x-ray images must be submitted in DICOM format, which is the international standard electronic file format for medical imaging. DICOM format is used for all imaging CDs and DVDs provided by imaging providers and medical facilities. Our uploading tool will only accept images in the DICOM format, so please do not send pictures of images taken with phones, tablets, etc. as the tool will NOT transmit those images to The Clinic.

We receive hundreds of requests for consultations and ask our prospective patients to send us their imaging via our online tool or mail so we may store all patient imaging files in one place, organized and accessible for any potential future care. Please do not send any viewing links/webpages from your imaging facility/radiologist as it will delay or prevent your consultation.
