Although there is no replacement for an in-person clinical evaluation, out-of-state patients may benefit from an outside consultation. Dr. Stone will phone or e-mail you to discuss your case and which, if any, surgical procedures he feels you may be a candidate.
 
Our Patient Coordinator, Luis Galvez, will be helping you through this process and can answer any questions you have regarding outside consultations. You can reach him at (415) 563-3100 and luis@stoneclinic.com.
 

To give a competent outside consultation, there are some items need first. We must have both current x-rays and MRI with typed reports. Dr. Stone is not able to do the outside consult without these images and reports.

When requesting an outside consult, please include:
 

•    SIGNED CHECKLIST – Click here for printable page.

•    COVER LETTER - State main complaint and bullet points of history, any specific
     questions you would like answered by Dr. Stone, and your phone number(s). Phone
     calls are typically placed between 4:30 and 7:00 p.m. on Mondays and Thursdays.

•    RECENT MRI FILMS and Typed MRI REPORT - Taken within the past year. If you
     have had surgery in the past year, the MRI must have been obtained after surgery. If
     you have not had an MRI and if you are planning to visit our office, we prefer to
     obtain the MRI here on our machine using our techniques.

•    X-RAYS and Typed X-RAY REPORT – Taken within the past year. If you have had
     surgery in the past year, the films must have been obtained after surgery. It is
     extremely helpful and preferred to have the following series of x-rays: PA flexion
     (knees flexed 30°) both knees, AP standing both knees, lateral standing of affected
     knee, cross table lateral of affected knee and skyline, sunrise, or merchant view of both  
     knees.

•    OPERATIVE REPORTS, PHOTOS, or VIDEO of prior surgery. This information is 
     helpful if available.

•    PHYSICIAN REPORTS pertaining to this specific problem. This information is helpful
     if available.

Please note, if you have a knee injury, refer to the Knee Imaging Views form (PDF) for the precise images we'll need in order to render an accurate diagnosis.

All items must be sent in one package with the patient’s name clearly marked on the outside of the envelope. Please mail information to:

Also, please indicate in your cover letter the method you would like to have your items returned. Preferred method is for you to provide a Fed Ex number on a self-addressed Fed Ex shipping label. Otherwise, you should prepare a return address label and enclose sufficient postage to be applied to your envelope. We cannot be responsible for lost films via U.S. mail, etc.

There is no fee to do the outside consultation, however, you may include a donation to the public, nonprofit Stone Research Foundation. Donations are used to support research and ensure that you receive the best possible orthopaedic care, today and in the future
