While there is no replacement for an in-person clinical evaluation, patients who live outside of the San Francisco Bay Area may benefit from a telephone Outside Consultation. Dr. Stone will call to discuss your case and which, if any, surgical procedures he feels you may be a candidate. Our Patient Coordinator will be helping you through this process and can answer any questions you have regarding outside consultations. Please watch the brief video above to learn more.

The following items are "required" for an Outside Consultation.

	Checklist for Outside Consultation Please print and complete the “Checklist for Outside Consultation” by clicking here. You must submit ALL of the listed items to have an Outside Consultation with Dr. Stone. Missing items will result in a postponed Outside Consultation.
	Recent X-rays and MRI Films Including Reports These films MUST have been taken within the past year. If you have had surgery in the past year, the X-rays and MRI must have been obtained AFTER surgery. The X-rays and MRI images Dr. Stone needs for your evaluation can be seen or printed here. If you have not had recent images and if you are planning to visit our office, we prefer to obtain the images on our machines using our techniques.
	Cover Letter State your main complaint and bullet points of the history of your injury as well as any specific questions you would like answered. Optional: Any relevant physician reports, operative reports, and photos or videos of prior surgery are helpful if available.
	Contact Information Please include your phone number(s) and a good time to reach you depending on your time zone. Once we have received your Outside Consultation packet, our patient coordinator will contact you to schedule your Outside Consultation with Dr. Stone, which typically takes place between 4:30 and 6:00 pm PST on Mondays and Thursdays.

Please mail ALL of the above items in one package to: The Stone Clinic Attn: Outside Consultation  3727 Buchanan Street, Suite 300 San Francisco CA 94123 It is important that you indicate on the Checklist the method you would like to have your items returned. Preferred method is for you to provide a FedEx number on a self-addressed FedEx shipping label. Otherwise, please prepare a return address label and enclose sufficient postage to be applied to your envelope. We cannot be responsible for lost films via U.S. mail, etc.

Via the Internet

The required items stated above may be submitted via dropbox.com To submit via Dropbox you must follow these five steps: 1. Create a dropbox account and read the instructions for setting up a public folder 2. Create two separate folders within your public dropbox folder titled "Images" and "Patient Information" 3. Within the Patient Information folder, upload your cover letter, radiologist reports, and contact information 4. Within the Images folder, create two separate folders titled: "Xray" and "MRI" and place the entire file contents inside. 5. Once everything is uploaded, right click on your public folder and "share" your folder with info@stoneclinic.com 6. Congratulations, you're done! Our patient coordinator will contact you to schedule your Outside Consultation.

There is no fee to do the Outside Consultation; however, you may include a donation to the public, non-profit Stone Research Foundation.
